Library Instruction Request Form

Schedule a session with a librarian so that your students can begin to learn the research and information literacy skills necessary to succeed in the university environment and beyond.

Please keep the following in mind when requesting your session:

Please complete one form per course section and submit each form at least one week in advance of your proposed meeting date to ensure the availability of a librarian, access to an instruction room, and preparation of materials.
Please schedule your session in conjunction with a research assignment. Your students will learn more if they have a research need when they come to class. Students should have chosen their topic before the session.
Please submit a copy of the assignment at the time of your request. You may submit your materials as an email attachment, or send them through campus mail to Harold Goss, University Library. If you would like assistance developing assignments, please use the Library Assignment Consultation Form.
Please schedule your session when you can attend.
